If you check the "Yes" box, to have your name placed on the Holler of Fame listing, then whatever name you enter will be listed. If you include your year of birth and/or country of residence, those too will be included in the list.
The Holler of Fame, first name, last name & email fields are mandatory. After you've paid the registration fee, you'll return to a page that will provide you with a download button for your certificate. We also will send you an email, to the address you've entered on this page. That email will include your personal, unique link to this site that will recreate this certificate and offer you the option to download it again if you need to do so in the future.
